Each service can be commissioned on its own or combined into a longer-term rehabilitation pathway.
Specialist Rehabilitation Assessment
A comprehensive assessment of functional presentation, needs, goals and priorities, providing a clear clinical foundation and recommendations.
Specialist Rehabilitation Programme
A focused programme around defined goals, involving one or more professionals including OT, Physiotherapy, SLT, Psychology and Rehabilitation Assistants. Short-term or longer-term, depending on the client’s needs.
Multidisciplinary Rehabilitation Programme
Two or more disciplines working around shared goals, with communication and clinical reasoning coordinated across professionals.
Suitable for: goals that need input from more than one discipline.
Complex Rehabilitation Pathway
A longer-term pathway for complex or evolving needs, focused on ongoing coordination, clinical review, adaptation and management of the wider rehabilitation journey.
Ongoing Rehabilitation Review
Structured reassessment of needs, progress and future priorities, following any assessment, programme or pathway.

Referral and Triage
Referrals are accepted from clinical professionals, legal teams and case managers. Our triage process ensures the best clinical outcomes for our clients.
- Initial referral screening within 48 hours of receipt.
- Specialist triage by a Senior Rehabilitation professional.
- Review of clinical records and expert reports.
- Assessment of rehab potential and goal setting.
- Recommendation for specialist multi-disciplinary input.​​

Communication and reporting
Commissioners and referring professionals can receive appropriate updates throughout the rehabilitation pathway, which may include:
- • Initial assessment report
• Rehabilitation plan
• Progress updates
• MDT updates
• Formal review reports
• Recommendations for future rehabilitation
• Discharge or closure summary
Reporting arrangements can be agreed according to the requirements of the commissioned service.
Working with the wider professional network
Rehabilitation often sits within a wider network of health, social care, family and community services. With appropriate consent, we communicate with relevant professionals to support continuity and reduce duplication, including:

• Employers or education providers where appropriate
